Traditionally, a dissertation is a single-author work. If you explore the recently submitted PhD dissertations in the OHSU Digital Collections, you will notice that the vast majority are authored by a single individual. In cases where there are multiple authors, the full records indicate that the additional authors are listed as contributors. Please ensure that your co-authors are designated as contributors.

We have seen a few examples of students who prefaced previously published works within their dissertation body with statements indicating that these works were adapted from manuscripts. However, it appears that most students have not utilized this specific wording.
